2. Identification code

3. Conclusion or another document confirming the need for rehabilitation aids

4. Disability certificate
   ➣ Certificate for the MSEC examination report

5. Birth certificate (in case of prosthetics for a child)

6. Pension certificate (if available)

7. Power of attorney, notarized (if the interests are represented by another person)

8. Application to the FUND
   ➣ for the provision of prosthetic and orthopedic products for a person with a disability
   ➣ The application form is provided at the social protection department

9. The application must include:
   ➣ all necessary rehabilitation aids
   ➣ the chosen prosthetic company — LLC “Ukrainian Center for Rehabilitation and Prosthetics ‘ZDOROVIE'”

Required documents:

1. Appendix 4 – VLK Certificate (issued at the hospital by the VLK commission)

2. Appendix 5 – Certificate of injury circumstances (wound, concussion, mutilation) (issued by the military unit)

3. Conclusion on the provision of auxiliary rehabilitation aids is issued by:
   ➣ a multidisciplinary rehabilitation team (MDRT)
   ➣ an expert commission
   ➣ a medical advisory commission (LKK)
   ➣ a military medical commission (VLK)
   The conclusion can be obtained already during treatment in a medical facility

4. Passport
   ➣ Extract of residence (CNAP or Diia)

5. Identification code

6. Combatant’s certificate (if available)

7. Application specifying the rehabilitation aids.
   ➣ Indicate the prosthetic company — LLC “All-Ukrainian Center for Rehabilitation and Prosthetics ‘ZDOROVIE'”

➣ Bathing prosthesis
➣ Assistive devices for personal care:
➣ 2 silicone liners
➣ A set of covers
➣ Orthopedic shoes
➣ Prosthesis repair
➣ Other TSR — as prescribed by a doctor
